IMMIGRATION INFORMATION

Please review this information carefully as it will help provide a better understanding and experience during your visit to our office. This will also aid
us in processing your paperwork successfully.

When you attend your office visit, please submit all documents and records so we may avoid any mistakes or delays of your application. Please bring an
interpreter with you if you do not understand or speak fluent English. This will ensure that we avoid unnecessary mistakes and delays in your paperwork.

These are the following documents needed for your visit:

  • Passport, driver’s license or another form of photo identification
  • A history of all of your completed vaccinations and testing documents, including COVID-19 vaccines. Please note, vaccinations cannot be older than 10 years old, which includes childhood vaccines
  • If you started your I-693 form, please bring it with you. If you do not have this paperwork, it will be available to you at the time of your visit.
  • Because we do not bill insurance, we can accept cash only for your visit. However, we will provide you with a receipt if you would like to submit it to your insurance company for reimbursement.
  • The basic cost of your immigration will include the initial doctor visit, processing your paperwork, and providing you with copies for your records and for Homeland Security. Depending on your vaccination and testing requirements, the total cost will vary. Please telephone for current pricing as they are subject to change.
  • If you have any symptoms of cold, flu, COVID or just feeling unwell, please wear a protective mask while visiting our office.
