IMMIGRATION PHYSICAL

Welcome to the Raffee Medical Group,
To apply for a U.S. green card (lawful permanent residence), an applicant must obtain and immigration medical exam. This information is intended to help you prepare for and complete your immigration medical exam.

What you must bring to your immigration medical exam.

  • A Government issued photo identification card (Examples: valid, unexpired passport or
    driver’s license or another form of photo ID)
  • Vaccination records, translated into English and in printed form
  • I-693 Form or we will provide one for you

Filing your I-693 form:

  • Carefully read all instructions in the form
  • Contact a civil surgeon (The Raffee Medical Office) to schedule an appointment
  • Fill out Part 1 of the form. DO NOT SIGN THE FORM
  • Attend your medical exam appointment and all follow-up exams that you may be required. When your civil surgeon is ready, they will ask you to sign the form in front of them. Upon completion of the examination and when all required lab results and requested documentation are obtained, you will be provided with a sealed envelope containing your completed Form I-693 and medical exam paperwork. DO NOT OPEN THIS ENVELOPE.
    We will provide a copy for your records.
  • Submit the Form I-693 in the sealed envelope to the U.S. Citizenship and ImmigrationServices (USCIS)

What Forms of Payment Are Accepted in Our Office?

  • Unfortunately, due to multiple instances of fraud and the very limited relationship with our patients we have had to adjust payments to cash only payments. We recognize this is inconvenient and therefore want you to know about this before your visit so appropriate arrangements can be made. Thank you for your understanding regarding this matter.

Who pays for the immigration medical exam?

  • The applicant is responsible for paying all costs of the medical exam.

Is payment due at the time of the exam?

  • Yes. Payment is due at the time of the exam. Payments should be made directly to the medical office via CASH ONLY. No checks, credit cards, debit cards or money orders will be accepted. Please come prepared with your CASH payment.
  • The basic cost for your visit of $250.00 will include your civil surgeon visit, (time spent with the civil surgeon) and processing all of your paperwork. This also includes a USCIS sealed envelope and a copy of the I-693 packet for your personal records.
  • Depending on your individual vaccination and testing requirements, the total cost willvary. Please telephone for current pricing as they are subject to change.

Is payment due at the time of the exam?

  • Less then 2 years of age:
    ◦ Physical examination
    ◦ Immunization review
  • Ages 2-17:
    ◦ Physical examination
    ◦ Immunization Review
    ◦ Tuberculosis blood test
  • Ages 18 and older:
    ◦ Physical examination
    ◦ Immunization review
    ◦ Tuberculosis blood test
    ◦ Syphilis blood test (ages 18-44 years)
    ◦ Gonorrhea urine test (ages 18-24 years)

What does a physical examination include:

  • It includes a physical examination that involves, at minimum, an examination of the:
    ◦ Eyes
    ◦ Ears
    ◦ Nose and throat
    ◦ Extremities
    ◦ Heart
    ◦ Lungs
    ◦ Abdomen
    ◦ Lymph Nodes
    ◦ Skin
  • There will also be a mental status evaluation that includes, at minimum, an assessment of:
    ◦ Intelligence
    ◦ Thought
    ◦ Cognition (comprehension)
    ◦ Judgment
    ◦ Affect and mood
    ◦ Behavior

What should I know about the Tuberculosis, Syphilis and Gonorrhea Tests?

  • Tuberculosis blood test must be provided by a civil surgeon. All people with a positive test will be required to have a chest x-ray to check for active TB disease in the lungs.
  • Syphilis (RPR) blood test must be provided by a civil surgeon and blood test results are generally available in 72 hours if results are normal. All people age 18-44 will be tested for syphilis (RPR). Applicants younger than 18 years of age may be tested if the civil surgeon assesses any risk factors for syphilis. Anyone with a positive test result must receive treatment from the civil surgeon.
  • Gonorrhea urine test must also be provided by a civil surgeon. Urine collection results are generally available in 72 hours if results are normal. All people age 18-24 will be tested for gonorrhea. Applicants younger than 18 years of age may be tested if the civil surgeon assesses any risk risk factors for gonorrhea. Anyone with a positive test result must receive treatment from the civil surgeon.

Do required vaccines vary by age?

  • Yes. USCIS requires documentation of immunity/vaccinations to certain diseases, depending on the applicant’s age. Applicants are to bring in their vaccination records. If you choose to go to another doctor or clinic for these services, you must bring your printed results to our office to complete the examination form. We cannot complete your packet until we have all necessary documentation from you, the patient.
  • Note: If the vaccine required is a series vaccination, only the first in the series is required at the time your application is signed and submitted.
  • If further proof of vaccination by either blood test or vaccine administration is required, we can provide these services for you.

Should I bring an interpreter to my exam?

  • Yes, if you are unable to speak, read or write in fluent English. If you require an interpreter, this person must be able to read, write, and speak fluent English. Even children can provide interpretation services at an immigration exam if they can read, write, and speak fluent English. However, if you do not have an interpreter, we do have an electronic translating device that may be of help.
  • Note: The interpreter will be required to fill out the Interpreter Section of the I-693 form at the time of the visit.
